Yep, I usually just don’t play for anyone 
This happens when I try to record as well, but not as bad.
When I watch a video of Kiyoshi cursing at the computer while doing many takes, I feel a kindred spirit whenever I try to record something XD

I have massively huge anxiety about playing in front of others and recording on video. Even more so with sax. This forum here cured me of the recording thing on bass (sax is still really anxiety filled but slowly coming) and all I can say is it gets better and easier but that damn record button still F’s me up almost every time.
Last week I covered a song on video on take #1 for the 50 song challenge! I was beaming for days. Progress.
I recorded something and was asked ‘how many takes?’ I said ‘First try’. She smiled and said ‘Lego Batman first try?’ Yep nailed it!
